Make A Referral

To apply for home delivered meal service, or to refer someone to us, call our office at (609) 695-3483. New client referrals come from visiting nurses, discharge planners, social workers, doctors’ offices, county social service offices, other agencies and programs, families and clients themselves. You will be asked to provide basic information about the applicant over the phone. Our Director of Client Services will then contact the client to set up an in-home assessment interview, and confirm the client’s diet with his or her physician, prior to the start of meal services. The following list contains information requested to be given over the phone:

Address (including zip code and apartment number if applicable)
Telephone Number
Date of Birth
Health Concerns of Client
Dietary Restrictions (note: our program does not accept clients with life-threatening food allergies)
Primary Doctor
Emergency Contact (name, phone, relation to client)
Referrer’s Information (name, agency/position, phone)
